The Case Manager facilitates the strategic planning process (Child and Family Team) for assigned children and families. Engages families, stakeholders, informal supports and others involved in the child/family's life for the primary purpose of developing a strengths-based plan, which meets the family's needs. The Case Manager I position ensures team participation and ensures relevant expertise is represented on the team to develop and implement an Individual Service Plan (ISP) for each child/family. Responsible for all CFT Process related documents including: Strengths, Needs and Culture Discoveries (SNCD), Individualized Service Plans (ISP), and support plans. In partnership with supervisors, agency staff, and administration the CM I is responsible for identifying and resolving any barriers that are impeding family's needs from being met. Primary Duties of Case Manager: * Facilitates CFT meetings for assigned families. * Facilitates the development of ISP's using information gained from the comprehensive assessment, the SNCD and input from all team members involved. * Provides information, education and support to youth/ families and team members concerning issues of creative treatment planning, coordination of care and the development of informal and community supports. * Ensures team meets as frequently as necessary in the family's preferred natural setting to meet their needs. * Develops, implements, and revises as necessary, a relevant crisis and safety plan for the family. * Ensures appropriate clinical liaison/therapist involvement on teams. * As CFT facilitator, works with family to achieve independence. * Provides information to CFT members concerning community resources and linkages to other services. * Assists youth and families in securing needed services. * Maintains accurate, thorough and current documentation of contact with youth/families, treatment progress and services received/provided, and accurate progress notes of all serious incidents and changes in behavior. * Ensures confidentiality of verbal and written youth/family information, in accordance with HIPPA standards and network policy. Skills / Requirements
Bachelor's degree in a social science or equivalent combination of education, training and/or experience.
Must be able to work independently with discretion and judgment to conduct case management functions for assigned families. Participates in individual and group supervision. May work directly with any SWN staff and may receive guidance and direction from SWN Clinical staff and/or Executive Team. Knowledge of community based behavioral health services preferred. Must have knowledge of spectrum of mental illnesses, DSM-IV'TR codes and descriptions, psychotropic medications and crisis intervention. Must be able to function as a member of the SWN case management team who facilitates CFT's for assigned families. Must maintain good consumer relationships, possess excellent verbal and written communication skills, and be able to identify priorities and stay organized. Important Notes
