At Southwest Network we are very proud of the work we do and our results are a direct reflection of our pride. We currently have an exciting opening for a System of Care Manager.

The System of Care Manager is responsible for overseeing case management teams and the implementation of child and family team practice. Provides oversight of team leaders and ensures Southwest Network children and families with high needs are being served accordingly. Attendance at child and family teams (as appropriate) and coordination with agency staff to ensure individualized plans are developed and implemented in a timely manner. This position is responsible for supervision and oversight of the Team Leaders. Position also conducts supervision, group consultation, clinical oversight, and provides leadership to the teams to ensure practice is within the parameters of the SWN service delivery model as well as the SWN model of practice. Oversees SWN staff efforts to prevent and reduce out of home placements to assure appropriate discharge planning. Assist staff in identifying and removing outpatient barriers to successful and timely service.

Works directly with the VP of Children’s Services, Medical Director and Clinical Care Director on ensuring individualized planning and interventions are being implemented for enrolled youth and families. Position is responsible for ensuring documentation and productivity is in alignment with SWN expectations. This position will provide technical assistance, training, clinical consultation/direction to Child and Family Teams, and assist in obtaining community and informal resource information. Also this position will coordinate with various Stakeholder groups, attend RBHA and other assigned meetings, and participates in Network Leadership Meetings.

Skills / Requirements
Knowledge and Ability:
Knowledge of community based behavioral health services and working within Child and Family Teams. Must be able to function as a member of a Child and Family Team with the Arizona Model and RBHA structure. Requires an understanding of both the philosophical and technical approaches to the Arizona Model. Use of creative solutions when providing clinical consultation/direction to Child and Family Teams is required. Additionally, the position requires the ability to motivate staff while monitoring and ensuring productivity is at or exceeding SWN expectations. Must possess strong leadership, coaching, and planning skills. Must demonstrate critical thinking skills and have the ability to monitor effectiveness of individualized service plans through use of youth/family directed data. Also requires an understanding of how to build off individuals’ strengths and continue to develop individual learning areas. Candidate must have sound working knowledge of administrative as well as practice-related supervision. Must maintain positive youth/family relationships, possess excellent verbal and written communication skills, and be able to identify priorities and stay organized. Must possess training and knowledge of supportive behavioral intervention techniques.

Requires knowledge of and experience with administrative operations, processes and procedures, involving: (1) Superior organizational skills with the ability to work independently and under pressure; (2) Excellent problem solving skills; (3) Excellent customer services skills; (4) Detailed understanding of community resources, eligibility and entitlement funding sources.

Education and Experience:
Bachelors Degree in Social Sciences with four years of clinical/case management experience working with youth/families in the behavioral health field. Training required in Non-violent crisis intervention, CPR/First Aid, and understanding of Parenting Dynamics and Child Development Issues.
