WANT TO MAKE A DIFFERENCE IN THE LIFE OF A CHILD WITH DISABILITIES?
Because our programs continue to grow, including new AzEIP contracts, we are currently looking for Physical Therapists to provide assessments and direct services to individuals with disabilities. Essential Responsibilities: * Provide assessments and direct therapy in individual sessions in consumers' homes or at the main site. Participate with teachers and other therapists in extending therapy into other program services. * Develop a therapeutic approach that can be carried out in the home, which relates to daily living with consumers, families, and/or caregivers. Assist consumers, families, and/or caregivers to follow through on programs developed. * Work as part of a comprehensive transdisciplinary team on the implementation of individual goals and objectives for each consumer. * Complete evaluations, contact notes, IFSP/ISP's and all written documentation as needed. In conjunction with the staff, consultants and referral sources, develop prescriptive therapy programs for individuals. * Identify needs for and assist in training in the use of augmentative devices and adaptive equipment. * Coordinate therapy programs and services with other community agencies and professional groups. Skills / Requirements
* Master's degree in Physical Therapy, with one or more years of experience working with children with developmental or multiple disabilities.
