Jobing Description
GENERAL POSITION SUMMARY:
The Intensive Case Management (ICM) Case Manager is responsible for the facilitation of Child & Family Teams for children with high needs.

DUTIES & RESPONSIBILITIES:
Develops and monitors Strengths and Culture Discovery summaries, treatment plans, and crisis plans with clients and participating agencies.
Assists clients in securing a variety of social services.
Maintains client records in accordance with regulatory, contractual and agency requirements.
Provides complete documentation and reporting of all client services in accordance with regulatory, contractual and agency requirements.
Meets with Child & Family Teams to review utilization of all services, both formal and informal.
Monitors client progress towards treatment goals.
Attends and participates in team meetings.
Participates in ongoing program planning and development.
Performs other duties as assigned, which are consistent with the general responsibilities of the position.


REQUIRED COMPETENCIES:
Demonstrates advanced knowledge and application of human development and family life cycle in children and families.
Knowledge of DSM-IV.
Knowledge of managed care systems, VO systems, and AHCCCS/Title XIX medical Assistance screening.
Knowledge of community resources.
Must be organized and able to work in a diverse, multi-cultural environment.
Effective written, oral and interpersonal communication skills.
Must be organized with the ability to take direction, prioritize tasks, multi-task, and work independently.
Ability to apply imagination and creativity to job duties and responsibilities.
Must have ability to maintain confidentiality, and consistently exercise discretion and judgment.




Skills / Requirements
MINIMUM QUALIFICATIONS:
Must be 21 years of age (licensing requirement).
Bachelor’s degree in Social Work or related field of study
2 years experience in child welfare.
Possess a valid Arizona driver’s license and be insurable under the agency’s automobile policy.
Must be computer literate with knowledge of Outlook and Microsoft Office programs (i.e. Word, Excel).
Must be able to provide own transportation to and from clients’ homes and/or other relevant organizations/agencies.



OTHER REQUIREMENTS:
Must be able to provide DPS fingerprint clearance.
Must be able to meet training and agency compliance requirements for the position.

Mental Requirements
Ability to effectively communicate in the English language.
Ability to interact well with various levels of staff, consumers, and with guests of AzCA.
Ability to read, comprehend and interpret complex information.

Physical Requirements
Office environment with frequent sitting, standing, writing and computer usage.
Must be able to climb stairs, with frequent standing and moving throughout facility or in the alternative, perform these physical requirements with or without reasonable accommodations.
Frequent to moderate physical effort (lift/carry up to 25 lbs.).
Moderate reaching, bending, stooping, kneeling or crouching.

Important Notes
Must be able to pass a fingerprint clearance.
