Job: Caregiver/Companion (Central, East & West Valley)

STATEMENT OF WORK:
Care By Design Associates will provide fellowship, care, and protection for the person who, because of advanced age or physical or mental infirmity, cannot care for his or her own needs. They will work under the direct supervision of the Scheduling Supervisor. Tasks are planned, documented and assigned according to the Client’s Care Plan to help meet the physical and emotional needs of the client. The associates perform any other job related tasks deemed necessary and/or assigned by the Scheduling Supervisor.

DUTIES AND RESPONSIBILITIES:

PERFORMED BY ALL CARE By DESIGN ASSOCIATES -
• Provide basic and immediate needs for CBD Clients.
• Assist with weight bearing ambulation.
• Provide minor emergency services; such as, caring for cuts, scrapes or skin tears.
• Run errands, do shopping, or provide transportation for appointments.
• Medication reminders. MEDICATION ADMINISTRATION IS PROHIBITED.
• Report any changes in Client condition to the Scheduling Supervisor.
• Be compassionate, courteous and positive at all times with Client.
• Perform any other job-related tasks deemed necessary (stated on Care Plan) and/or assigned by the Scheduling Supervisor.
• Complete required documentation in a neat, timely, and accurate manner; obtaining signature from an authorized person after services have been rendered.

DUTIES AND RESPONSIBILITIES:

CARE By DESIGN ASSOCIATE - COMPANION:
IN ADDITION TO DUTIES STATED ABOVE:
• May serve meals already prepared; must leave kitchen in orderly manner, clear table and wash dishes (Client only).
• Encourage exercise for Client.
• Engage in conversation, and/or games or puzzles if Client wishes companionship in these activities.
• Change linens as necessary for Client.
• Comb hair and assist with oral care.

DUTIES AND RESPONSIBILITIES:

CARE By DESIGN ASSOCIATE - PERSONAL CARE:
IN ADDITION TO DUTIES STATED ABOVE:
• Assist with meal planning and or meal preparation for Client.
• Assist with feeding if necessary.
• Prepare and assist with a type of bath; shower, tub, or sponge. Maintain Client
safety in and around bath area. Maintain a comfortable room temperature, free of drafts.
• Leave the bathroom and bedroom in an orderly manner after assistance.
• Provide skin care. Apply lotion and inspect skin for any open or bruised areas.
Report any abnormal abrasions to the Scheduling Supervisor.
• Assist with cleaning and filing of nails. CUTTING NAILS IS PROHIBITED.
• Assist with routine transfer and ambulation activities such as walking with walker, cane, or wheelchair.

KNOWLEDGE, SKILLS AND ABILITIES:
REQUIRED:
• Must be able to recognize unmet Client needs and potential problems and to notify the Scheduling Supervisor of these observations.
• Must be able to perform job with minimal supervision.

PREFER:
• Knowledge and understanding of personal care needs pertaining to the elderly and disabled.

WORK EXPERIENCE AND EDUCATION:
REQUIRED:
• Must be at least 21 years old.
• Must complete Care By Design orientation
• High School Diploma/GED.
• Minimum of one year professional caregiving experience working with elderly and/or the disabled.

PREFER:
• CPR and First Aid certification
• Ability to show evidence of being free of communicable disease (TB) upon hire and annually thereafter
